At Mission HHH, our home health program is designed to bring skilled medical care and personalized support directly to your home. We help patients recover, manage chronic conditions, and regain independence in a comfortable and familiar environment.
Our multidisciplinary team, composed of registered nurses, therapists, medical social workers, and home health aides, works closely with each patient, family, and physician to develop a care plan that promotes healing and enhances quality of life.
Nursing
Our caring, highly skilled nurses have extensive knowledge in educating and treating patients in the home setting.
Our nurses are trained in disease-specific education and management for conditions like heart failure, COPD, diabetes, wound care, pain management, and orthopedic rehabilitation. We also assist in medication management and administration, and provide dietary and nutrition management.
Through education and management, our Nurses help you and your caregivers live a healthy and independent life.

Physical Therapy
If there is a need for physical therapy, Mission HHH is an excellent choice. Our physical therapists are highly trained and skilled at helping patients improve and restore their physical function and mobility levels at home. Physical therapy can help patients who have difficulty walking, trouble balancing, require the use of adaptive equipment, or need rehabilitation post-operatively. Our therapists help patients post joint replacement, traumatic accidents, head injury, cardiac events, and strokes. They are trained in the latest therapies and can help you regain independence as quickly as possible.
Physical Therapy Plans can include:

Occupational Therapy
Our occupational therapists help restore a patient’s ability to return to their usual activities of daily living.
This may include a self-care routine, meal preparation, and maintaining their home independently.
Our therapists are able to help patients understand techniques in energy conservation while maximizing their fine
motor skills and regaining physical strength to help them accomplish daily tasks.
Occupational Therapy Plans can include:
- Restorative therapy for independence in daily living
- Home assessment for potential hazards
- Promotion of healthy living practices and social participation
- Schedule maintenance, routine building, and coping skills
- Recommendations for home modifications
Speech Therapy
Our speech therapists help patients improve their ability to communicate effectively and safely.
Therapy focuses on enhancing speech clarity, language comprehension, cognitive communication, and swallowing functions for individuals recovering from illness, injury, or neurological conditions.
Through personalized techniques and compassionate support, our therapists work closely with each patient to restore confidence and independence in communication and daily interaction.
Speech Therapy Plans can include:

Medical Social Work
Our social workers assess the social and emotional factors affecting each patient and their loved ones,
and then assist in coordinating available community resources and short-term counseling.

- Medical Power of Attorney / DNR
- Nursing home and institutional placement assistance
- Providing supportive case work services
- Development, implementation, review, and revision of the plan of care services
Home Health Aides
Our Certified Nursing Assistants (CNAs) work under the supervision of registered nurses and therapists to assist
patients to safely perform activities of daily living. Our CNAs are trained to assist patients with activities
of daily living such as bathing and grooming. Under the supervision of our nurses and therapists, home health aides
support the patient and family until independence is achieved.
Services can include:
- Bathing and Hygiene
- Assist with Ambulation and Transfers
- Light housekeeping pertaining to the patient
- Hair and Nail Care
- Dressing
- Linen change
The Home Health Aide Responsibilities do not include:
- Vacuum and mop the entire house
- Clean cupboards and closets
- Transportation
- Wash Windows
- Mow Lawns
